CO-morbid Long-term disease Management in the Americas: blended intervention (COLMA)

GENERAL OBJECTIVE: To develop and evaluate the implementation of an intervention that combines a digital health intervention (DHI) supported by community health workers with community support groups to improve outcomes for vulnerable individuals with comorbid depression, hypertension, and/or diabetes in primary care health centers in Peru and Argentina.
SPECIFIC OBJECTIVES:
1. To test clinical and implementation outcomes in the real world 12 months after implementing the digital health interventions (DHI).
2. To build capacity through scholarships and training opportunities in diverse disciplines.
